Job description

Key Responsibilities:

• Provide high-quality, patient-centered primary care to individuals and families.
• Diagnose and manage acute and chronic medical conditions.
• Perform routine health screenings, immunizations, and preventive care.
• Prescribe medications and coordinate follow-up care as necessary.
• Maintain accurate patient records in accordance with HSE and practice guidelines.
• Collaborate with practice staff, including nurses, administrative teams, and other healthcare professionals, to ensure continuity of care.
